One of our senior clinicians will review your request and we will respond to your request within 2 working days.
We may offer you advice, an appointment with one of the clinical team, or signposting to a more suitable team or service.
When you complete the online form, please give as much information as you can and please mention if you have multiple issues to address or require an interpreter.
This is to ensure you are booked into the most appropriate appointment type.
